About Us Emergency Travel Document

Requirements for getting an Emergency Certificate are the following:

  1. Completed, printed, and signed Emergency Certificate (EC) Application form: https://embassy.passportindia.gov.in/ or https://portal4.passportindia.gov.in/Online/index.html 
    1. Register first by choosing the Embassy/Consular as Colombia - Bogota and filling your details. You will get an activation email link to your email after successful signup. 
    2.  Activate the user by clicking on the link received on your registered email address and using the option User Login, login with your registered email Id/User Id and Password in the above portal. 
    3. After successfully logging to the portal, click on the option Apply for Emergency Certificate, and submit the application after filling all the details successfully.)
  2. Proof of your permanent residency in Colombian/Ecuador. (Copy of any public service bill)
  3. Recent proof of your place of residency in India.
  4. A letter containing detailed reasons why the person is applying for an Emergency Certificate (EC) including contact detailed information (email, home address, mobile number). Address with zip code that it can be found in www.codigopostal.gov.co or www.codigopostal.gob.ec 
  5. Three (3) photographs 5cm x 5cm 
  6. Copy of Lost/Stolen passport (Front and Back page of the Passport.) 
  7. Police complaint/FIR Report of the lost/stolen passport. 
  8. Copy of Highest educational Degree Certificate, PAN card/income tax card /Voter Identity card, driver's license, or any other certificate that can validate the applicant's birth date.
  9. Proof of work permit/stay permit/visa/copy of identity card showing the legal status of the applicant in the country of residence.
  10. Filled in Copy of the Personal Particulars Form (CLICK HERE) May please note to fill the Indian references in the References section (No. 10) in the form.
  11. Affidavit of lost passport, CLICK HERE for the format of the Affidavit.
  12. Confirmed airplane ticket with the itinerary details for return to India. (If transiting through any other country, the tickets should be booked in such a way that, the transiting country does not need a Transit visa.) Embassy needs at least 2-3 week's time due to lockdown and disruption in courier services etc for issuing Emergency Certificate so tickets may be booked accordingly.
  13. If sending the documents by Courier, 4-5 signatures of the applicant on a white A4 sheet with sufficient space between each signature.

Note: Please remember that the address in the application form SHOULD BE THE SAME to the address provided as proof of residency in Colombia and the address provided as proof of Indian address.

DHL(optional): Need to pay DHL/Courier charges in case if the applicants want the Embassy to send bank the documents/Passports through courier.
